Tokuda: “You conducted a shoddy investigation”

SA: …Former state Sen. Jill Tokuda, a Kaneohe resident, was incredulous that Suzuki and Anderson offered no details of how Saito got the means to escape.

“It’s absolutely unacceptable,” Tokuda said. “The Legislature was extremely patient with the administration — from the AG to the Department of Health. … But a response like this does not reflect any accountability. It doesn’t mean you should fire people. But to me multiple individuals assisted Randall Saito and should be held accountable. To tell us no one was helping him on the inside, then who was helping him on the outside? If you’re not able to answer those questions, then you conducted a shoddy investigation. Wow, it’s just so wrong.”

Lawmakers during last year’s legislative session took hospital officials to task over reports that Saito had a half-dozen sexual relationships while in the psychiatric facility — including with hospital employees.

House Health and Human Services Chairman John Mizuno said at a hearing in February that an evaluation of Saito in 2010 revealed he had six “significant relationships” while committed.

“There seems to be significant evidence that the person in question had sexual relations with at least three staffers,” Mizuno said. “That’s part of the reason how Randall Saito probably got his privileges and was able to escape. He got a lot of contraband, and this is a major concern.”

Surveillance video footage showed Saito taping the locks of doors in the facility, retrieving a bag with clothes and other items from a locked cabinet and walking off the campus after opening a combination lock on a gate….

ATTORNEY GENERAL AND DEPARTMENT OF HEALTH ANNOUNCE CONCLUSION OF ADMINISTRATIVE INVESTIGATION INTO ESCAPE FROM HAWAII STATE HOSPITAL

News Release from Office of Attorney General, Dec 12, 2018 

HONOLULU – The Department of the Attorney General and Department of Health announce the conclusion of the administrative investigation of the Nov. 12, 2017 escape of Randall Saito from the Hawaii State Hospital.

“The investigation was complex and required many hundreds of hours of review, evaluation, analysis and documentation by a committed team of investigators,” said Attorney General Russell A. Suzuki. “In keeping with Gov. Ige’s commitment to honesty and transparency, the report is being released so there can be a clearer understanding of the challenges faced by the Hawaii State Hospital.”

The investigation is fundamentally a review of personnel actions and protected by law from disclosure. However, Attorney General Suzuki has decided that for purposes of transparency, a redacted version of the final report will be released.

The detailed investigative report determined that no single, individual hospital employee was directly responsible for the escape. However, the report noted Hawaii State Hospital’s inability to adjust to a predominantly forensic patient population contributed to Saito’s escape.

“We knew the AG’s detailed investigation would take time, and we did not wait to take immediate corrective actions and make improvements at the hospital to ensure safety,” said Health Director Dr. Bruce Anderson. “Increased security measures were immediately put in place by the hospital administration, and we continue to make improvements that increase patient monitoring to protect employees as well as the public.”

During the investigation, six Hawaii State Hospital employees were placed on off-duty status. Two of the employees have retired and four will be reinstated to their positions and will be retrained. Based on the report findings, no hospital employees will receive disciplinary action.

KHON Aug 8, 2018: Attorney: Randall Saito got help from visitors at Koolau Clubhouse

KHON Aug 13, 2018: State to release logs of visitors to Randall Saito at Koolau Clubhouse--Since Saito's escape, the state hospital patients have not been allowed to go to the Koolau Clubhouse. It'll remain closed until the state finishes its investigation.

KHON Dec 12, 2018:  Suzuki did not offer any answers as to how this happened. He says there is a separate criminal investigation that is still ongoing, which will look into how Saito was able to acquire all those items which helped him escape. It's not clear when that investigation will be finished.

HGEA Laying Down on the Job:

HNN  Nov 14, 2017: Documents from 1993 obtained by Hawaii News Now revealed that Saito had sexual relations with at least three hospital staff members. Saito has been known for his manipulative nature, and sources say sexual relations took place on multiple occasions where he was able to obtain contraband.
