Public Required to Complete and Return Juror Questionnaires

News Release from Hawaii Judiciary August 1, 2012

As it does each year during the first week in August, the Hawaii State Judiciary mails juror questionnaires to persons who have a Hawaii state driver’s license and are registered to vote in the State of Hawaii. This year, beginning August 6, approximately 235,000 juror questionnaires will be mailed to 85,000 residents on Oahu, 55,000 on Maui, 70,000 on the Big Island and 25,000 on Kauai. The questionnaires are used to help select persons who may be eligible to serve as jurors in 2013.

This year, a new Certification of Disability for Disqualification from Jury Service form will be included in the mailout. In addition to the questionnaire, the Certification of Disability form should be filled out by persons claiming a permanent disability that prohibits them from jury service. One section of the form must be completed by a physician or physician’s assistant.

Those who receive a questionnaire have 10 days to complete and return the questionnaire to the Jury Pool Office. Anyone who fails to respond may be penalized.

To be eligible to serve as a juror, one must be at least 18 years old, a citizen of the United States, a resident of Hawaii, and able to read and understand English.
